मान्यता प्राप्त नर्सिंग कॉलेज से बी.एससी. (नर्सिंग) / पोस्ट बेसिक बी.एससी. (नर्सिंग) के साथ एकीकृत सामुदायिक स्वास्थ्य प्रमाणपत्र कार्यक्रम (CPCH); शैक्षणिक वर्ष 2016–2020 या उसके बाद उत्तीर्ण। 22.11.2026 को आयु: सामान्य/ईडब्ल्यूएस 35 वर्ष (विज्ञापन के अनुसार श्रेणीवार छूट)।
